Audio-only Telephone Communication

Although used by providers, this mode of telemedicine is very limited following the regulations provided by state law. 

Store-and-Forward

All health information shared through this mode can be in pre-recorded videos and digital images or photos. However, this excludes any radiology data. The transmitted information must be particularly related to the patient’s medical condition and would allow the physician who will receive them to provide a diagnosis and corresponding treatment plan adequately.  

Under the store-and-forward mode, it is the telemedicine provider at the distant site who will be reimbursed. However, the reimbursement is limited to 75% of the Medicaid fee for the SAF service and can only be claimed upon submitting a written report by the practitioner from the distant site to the practitioner at the originating site. 

This mode is often used in dermatology and ophthalmology, but other fields are also starting to utilize it. In teledental services, however, asynchronous or SAF service is not reimbursed; the actual procedure is. 

Remote Patient Monitoring (RPM)

Telemedicine services rendered through remote patient monitoring are reimbursable only when delivered by Certified Home Health Agencies (CHHAs) and other certified telemedicine providers. Regardless of what agency is involved, the following conditions must be strictly complied with: 

• The RPM services must be done on the order of a physician, midwife, or nurse practitioner. 

• There must be an existing and ongoing relationship between the patient to be monitored, and the health care providers stated. 

• An in-person interaction between the patient and practitioner at some point of the treatment, either at the initial stages or for follow-up purposes, is necessary. 

• There is a medical necessity for the monitoring, and when the necessity ceases to exist, it should also be discontinued.

Under New York Private Payer laws, telehealth is defined as any health care service rendered by a practitioner through electronic information and communications technology to a patient situated in a different site from the practitioner at the time of delivery of said service.
